Pre-registration is a function performed within the Access Services Department of Ball Memorial Hospital. To expedite registration, you have the ability to complete pre-registration through the power of the Internet. This Web Site allows you to complete the pre-registration process from your home and other locations with Internet access.

Why pre-register?
Pre-register for your service, test, procedure or surgery to expedite the registration process in order to decrease patient wait time. We know your time is valuable!

How is a patient pre-registered?
Please submit at least two days prior to your scheduled visit. This allows the staff adequate time to verify and pre-certify your insurance (if applicable). If you are unable to meet this timeframe, pre-registration can be completed by telephone the day prior to your outpatient test.

Pre-registration can be completed several different ways for the patient's convenience. Choose one of the following options:

• Complete the form
• Call the Access Services Department at 765-751-5075 or 1-866-477-3734.

Be prepared to provide the following:

• Date of test and procedure
• Demographic information
• Insurance information (on card)
• Complete physician names(s)
  

Hours for Pre-registration
The Access Services Office hours of operation are from Monday through Thursday 8 a.m.-8 p.m. and Friday 8 a.m.-6 p.m.

Is on-line pre-registration secure?
The Access Services Department supports the Ball Memorial Hospital Core Values by respecting the right to privacy and confidentiality of all patients. The information you submit will be transmitted using SSL security.

How will the patient know if the hospital has
received the pre- registration information?
If the Access Services Department has not received an on-line pre-registration prior to the scheduled test or procedure, the patient will be called to pre-register by telephone.
